چکیده

Background Bicuspid aortic valve (BAV) is known as the most common congenital anomaly and its complications (ascending aortic aneurysm, dissection) can lead to noticeable morbidity and mortality. There is increasing evidence that, in addition to its genetic background, changes in aortic hemodynamics may play an important role in the development of aortopathy in these patients. Studies have shown that BAV can substantially alter aortic hemodynamics, but it is unclear if the genetic basis can influence aortic properties and 3D blood flow in BAV relatives with normal tricuspid valves. In this study, we evaluated flow dynamics, aorta diameter and geometry in BAV relatives compared to normal volunteers using 4D flow MRI.

4D flow MRI demonstrates altered aortic hemodynamics in patients with right-left and right-noncoronary bicuspid aortic valve fusion patterns

Background Altered aortic hemodynamics associated with bicuspid aortic valve (BAV) have been implicated in the development of aortopathy in patients with congenitally altered valves[1]. Most studies have focused on the most common “right-left” bicuspid aortic valve (RL-BAV) fusion pattern [2]. This study assessed the effects of valve morphology on aortic 3D blood flow in cohorts with the most c...

From unicuspid to quadricuspid: the impact of aortic valve morphology on 3D hemodynamics

Methods In-vivo aortic 3D hemodynamics were evaluated by MRI in 17 patients with unicuspid (n=3), bicuspid (n=9, 3 true bicuspid, 3 right-left (RL) coronary leaflet fusion, and 3 right-non (RN) coronary leaflet fusion), trileaflet (n=3), and quadricuspid aortic valves (n=2). Valve morphology and dynamics were assessed using 2D CINE MRI. Aortic blood flow was measured using ECG and respiration s...

Phase contrast 4D flow in bicuspid aortic valves in a porcine model

Background Bicuspid aortic valves (BAV) are associated with aneurysms of the ascending aorta (AAA). It is unclear whether these aneurysms are caused by tissue alterations of the aortic wall or by alterations of blood flow in the ascending aorta due to the bicuspid valve morphology. In this study we analyzed the phase contrast 4D flow characteristics in normal tricuspid aortic valves (TAV) in a ...

Impact of Beta-blocker, ACE Inhibitor, and ARB therapy on thoracic aorta wall shear stress in bicuspid aortic valve patients

Background Bicuspid aortic valves (BAV) are associated with higher incidence of aortopathy such as aortic dilatation, dissection, and aortic valve stenosis. Recent 4D flow MRI studies have provided evidence that BAV aortopathy is associated with specific hemodynamic abnormalities and increased wall shear stress (WSS) which is thought to alter aortic endothelial cell function and induce remodeli...

Bicuspid aortic valve is associated with altered wall shear stress in the ascending aorta.

BACKGROUND Hemodynamics may play a role contributing to the progression of bicuspid aortic valve (BAV) aortopathy. This study measured the impact of BAV on the distribution of regional aortic wall shear stress (WSS) compared with control cohorts. METHODS AND RESULTS Local WSS distribution was measured in the thoracic aorta of 60 subjects using 4-dimensional (4D) flow-sensitive magnetic resona...
